What should we bring to doctor for medical check up in Toronto?

I did mine in North York. I brought 4 photos, printout of appendix C and my passport. The doctor who did my exam accepted cash only ($165). Everything went well and CIC received my medical results.

I had the same experience in Calgary, except my price was $180.00. The photo size was listed in Appendix C for me (American). I had extra photos just in case. They didn't require any former doctors contact information, prescriptions I was taking (she asked but didn't request to see the bottle), or really anything else. It lasted less then five minutes... I think the doctor in my case is more interested in collecting fees than truly evulating immigrants.
