Ours was not in Toronto, but a friend of mine's was. Her husband (from the US) needed 6 immigration photos, photo ID and about 400 dollars I believe.

I was meant to have my medical at Medisys Health Group in March before I left Toronto but the receptionist gave me bad information and said that I had to wait till I was notified by CIC to do my medical. She must have thought that I was applying for residency under a different route than Family Class.

In your situation I would call one of the doctor's listed above and they should be able to tell you what exactly you need. I know that the cost of the medical varies by Doctor so maybe call a few different ones.
 
The size of photos for medical application are same for applications size ? 35mm x 45mm ?
 
So should we send our sponsor application to Alberta or Ottawa ? I'm still confuse...

First question regarding photos, yes.

This question quoted above, depends on if you are applying inland, or outland. Also, it needs to be BOTH the sponsorship application AND the immigration application together. Inland, Alberta. Outland, Mississauga
 
I did mine in North York. I brought 4 photos, printout of appendix C and my passport. The doctor who did my exam accepted cash only ($165). Everything went well and CIC received my medical results.

we went on a saturday morning (we both work all week) and on saturdays they dont have appointments its first come first serve. we were first in the door at 9:00 am we were waiting outside at 8:50 with lots of others doing the same thing. xrays, dr visit and blood test are all done in the same building which is great. whole process too 1.5 hrs approx. super easy. we took the cash (just under $200) specified photos (as requested by region specific guide) and the paperwork for him to fill out. I would definitely reccommend him!

Did mine in North York also! The Dr. accepted debit but needed $30 cash for postage. The Dr. will send your results to the correct office. You send your receipt and confirmation to the visa office processing your application.

Midwest - can you tell me how long it too for CIC to receive your results? I had my medical 3 weeks ago now and nothing has been received as yet.

I don't really know, but once my hubby got approved as a sponsor and got UCI, "Medical results have been received" appeared in ECAS. I guess it took less than 3 months.

Yesterday I had my medical check up in Toronto for sponsor inland ( my wife sponsors me) . The doctor asked me many questions and when she asked me do you have high blood pressure ? I said Yes and explained in details . After that the doctor asked me the other question and when I said " No "..but she kept checked " Yes " ... I was confuse at that time and so silly why not asked the doctor why she still kept checked/ticked " Yes" even I said " No ".. I was afraid to asked because she is Immigration/ CIC doctor .
Is there anybody know the list of questioners ? Is that question related to medication of high blood pressure? She was fast asked the question and wrote wrong year but I told her that she wrote wrong year.
My other question is : Can i send my applications right now ? or should i wait for medical result first ? Should I give the copy of the doctor's receipt and X Ray receipt together with my applications? The next day I asked the receptionist about the questioner but she didn't know

I have asked this topic before but nobody answer my questions ...... :'(
