What name to use on my Conjugal Partner Sponsorship Application

My name in my Passport in my Married name ( Ma Jan Krizza Tamayao) but all other IDs and documents I have are under my Maiden name ( Ma Jan Krizza Guillermo). I am so confused right now on what name to use in fillin up the forms since all other documents I have is under my Maiden name whice is Ma Jan Krizza Guillermo. Is it okey if i attach an affidavit that will tell them that Ma Jan Krizza Guillermo and Ma Jan Krizza Tamayao is one person only. I only have the Civil Service Commission's declaration allowing me to revert my name back to Maiden and Special order here at work that I am using my Maiden name not my Married name. Please give me advice on what to do, Thankyou so much!

PS: My partner and I is Applying for conjugal partner sponsorship and Visitors Visa.

Yes but will it not confuse the Immigration Officer if the name I put in my Forms is my married name while the Documents attached are in my Maiden name.. Like the letters and tickets, bank accounts.

I am pretty sure there is/are section(s) in the application that you can mention ALL your names used.
