What if parents dont submit medical?

My parents applied for a visitor visa extension from within Canada. They plan to return on August 8th but yesterday we got an email from CIC asking for medical which is supposed to be submitted by Aug 17. My parents will have already left Canada by Aug 17th so there is no point in doing medical which is really costly. In this situation, what is my best option? Can they just skip medical and go back?
Any suggestion would be appreciated. Thank you.

If they stayed past their visa expiry date they have to do the medical because they took advantage of the time extension.
