How long it will take to rectify the names and resend the medicals request forms again by CHC, ND.

No need to send the forms back to Visa Office, send a mail explaining the mistake in the medical forms , they will send you reply regarding correction and take that to DMP. All this is done within 2-3 working days.

While doing the medicals, the DMP will have to fill in all your details in a fresh form. At that time, you can have the name corrected, any way you will be carrying a proof of identity when you go for medicals. dont worry about it.
