There hasn't been any refusal before.

It was declared that she was pregnant at the time and she was going to have the baby before traveling because delivery date is far from travel date
He means the CURRENT refusal letter not anything from before, was there any other information?
Did she apply herself?
Did she link the application and/or was receiving her updates through email?
If you used an agent they probably provided false information or documents because you generally get a chance to respond if they suspect you provided false info this is the "fairness letter".

Used an agent and agent is not declaring if they received a fairness letter or not.

There you go, he probably lied somewhere.
You should get not only the caips notes but also copies of all documents submitted for your application, the caips notes companies charge extra for this or find someone in Canada with citizenship or PR ans is willing to do it for you.

If the agent is not telling her that there was a fairness letter issued (which there has to be before any ban), then they have 100% provided false documents or information when submitting the application. That is why everybody that is using an agent should check and double check the application before authorizing the agent to submit in in their behalf.

If they have signed the use of a representative form designating the agent as the representative, they will want to cancel that ASAP. Use the same form (IMM 5476) to cancel the agents authority.

She provided all of the documents, just a few letters was written by the agent and the agent filled the online form also but paper submission was what was done

Unfortunately, you have been banned for 5 years. Your agent has not been truthful to you. S/he probably made false statements and/or submitted false documents as well.

Unfortunately, you have to pay the price for the mistakes made by your agent. While you can certainly apply for the case notes, these notes will not help to overturn the ban. IRCC will not accept the explanation that you were unaware of the Procedural Fairness letter and/or that your agent made false statements
