What documents should I submit to the VFS

I received my ppr, and I have to submit my passport. What documents including my passport should I provide them?
I read that Original Passport Request is required.
I don't know if there is any other document beside the consent form, but as I applied without an agent, what blanks should I fill in the Consent Form?

Passport Request Letter,
2 Copies of Passport Data Page,
The Original Passport for Stamping,
VFS Consent Form( You can get it on their Site),
Then write your name,address,phone no and email in a a piece of paper because that’s where they would send the passport to.

Fill the lower part of the form.
