WES for National university of Bangladesh

I am form Bangladesh.I completed my graduation Under National University.
I have some confusion on WES form fillup:

There is some field as like:

Year Issued *
Years Attended *


My session is 2000-2001 and Passing year 2004 in certificate and certificate date is 31/8/2014. But my final exam held on 2006 due to university session jam(failing to complete the scheduled exams in time).
So for my case what should be the "Years Attended" and "Year issued".

Please help.

Year issues is 2014 ; which is your certificate award date.
Year attended 2001-2004; it's when you start your class and finished your class. OR as mentioned in your certificate.
