WES ECA Received as " The names on the academic credentials submitted differ

Hi everyone,
I just received my ECA report from WES and it say as below:
" The names on one or more of the academic credentials submitted for evaluation differ from the name indicated above."

Please guide me what to do in this case. How to proceed on the process and ECA Score to be received by them for further processing of my PR to Canada.

The gist of it is, you have to submit an affadavit signed by a notary to say that the name in the certificate is basically you. You can find some samples in the link above or you can google" one and same affadavit".
