Visitor extension rejected - question

Hello,

a few months ago my husband applied for an extension on his visitor status, but yesterday we received
a letter from the processing centre in Vegreville, saying he did not provide proof of entry.
We thought he had given them the proper documents, someone please explain to us what they need for this?
This is what the letter states:

"..You have not provided proof of entry into Canada as required.
You have indicated that your date of entry was 10OCT2010. Based on this information, it is possible that your temporary resident status was valid until 10APRIL2011.
We also wish to advise you that your temporary resident status EXPIRES on UNKNOWN AS NO PROOF OF ENTRY WAS SUBMITTED."

If it makes any difference, he applied online on the government site. Please help us if you can.

A copy of the marked pages of his passport (entry into Canada) or copy of the visitor record should have been attached to the application.
