Vaccination Record form for Immigrant from UAE

Hi All,

I need some help and information from my fellow forum friends.

I would like to know if there is an immunization record form i can download from the internet? My wife and kids are heading to Toronto in a weeks time, and i remember the agency that assisted in our immigration, gave us an immunization record form along with our landing documentation, and now i seem to have misplaced it and was wondering if any one of you can guide me as to where i can obtain this?

If there is a previous forum thread similar to my query, i would be happy if you'll guys could point me in that direction.

I found one here: http://www.york.ca/NR/rdonlyres/fu3subjaurp5kkgyk7hkgvveim2y7hz7mc7c7bfndsruav3zpgm5hcciqughjkvfa2dv7476ts23coolqs3ck6cybf/Immunization+card.pdf but I don't know if the school will accept it if you have filled it out yourself. You can go to a doctor when you arrive and get them to fill one out for you.
