Using common-law declaration form for BOWP

Hello,

I know there was one topic similar as my question, but i couldn't find right answer. and CIC call centre won't answer today, so i am asking you guys again.

We are all ready to send out our application form for BOWP, but statutory declaration of common law form get notarized..there was no one could do it for 3 weeks.

and yes of course we don't want to wait till then...

so my question is, we already had the form notarized once to send his work permit extension before in novermer 2014.
do you think it would be still ok to use this time?

If anyone had a same situation, let us know please, so we can send this thing out then sleep happy.

The one the used previously, but the date under length of common-law relationship is previous date. (November: when we got singed)
Can i still use it?

I don't see why not,it doesn't expire.
I'm using the same declaration that I used for my PR app back in May 2014 for my BOWP application.
