Use of representative form, please help.

Hi,

My agent says tht only CIC registered agents can use the 'representative form' while filing.
In my case my LOA and Fee reciept has agent's name and address. Also in the student questionnaire I have mentioned about agent.

Please guide, should I add representative form or it is ok??
