i just want to know whether it is compulsory to include academic records request form while applying for ECA as my university ( Osmania University) is providing only transcripts in the sealed envelope.

I took transcripts from my college and submitted it to my university along with the academic transcript form. Yes it is sort of required. In fact my university personnel readily signed the stamped academic request form in front of me.
