URGENT PGWP HELP!!!!

Please help me with this matter. What date should I put in the 'from' section of current country or residence in PGWP form. I first got one year study permit for a two year diploma, then applied for extension than had to apply for an amendment for the extension( the condition only allowed me to work on campus). But now what date do I use in this section of pgwp. Issue date of first study permit or issue date of latest study permit? Please any response is appreciated.

Yes, PGWP is once in a lifetime.
