Hi Folks,

Just received an email from CIC to send Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature is required covering the past 10 years, without leaving any gaps in time. I hope this request maybe due to change in my status in last 2 months. I had resigned from my previous employer (the one qualifies for CEC experience) and incorporated my own company and doing contract jobs. Last month I had updated my change of job to CIC over email and got acknowledged with email confirmation.

Now I am quite confused what details do i need to input in IMM5669E application under Personal History (Work Experience) for the last two months, my questions are as follows:

1) Do i need to show my incorporated company name? OR
2) Do i need to enter the contracting company name which i have signed an agreement to provide my services?

Could anyone please clear my confusion.

You would either write "self-employed" your or company's name. You wouldn't write a client's name.
