Dear members!!

What forms are needed to be filled when a baby is born and what are the other requirements???

This is what I did last year and it got processed.

First you would inform them just after birth. VO will ask following, either together or in bits :

1) Birth Certificate of new born from Hospital.
2) Local Govt body registration (In my case it was registration with Cantonment board as hospital was with Cantt)
3) Nadra Registration/Update of Form B
4) Passport for new born after you register with Nadra
5) Processing Fee (Bank draft) for newborn - CAD 165 (please check if it is revised)
6) VO may ask you to update Principle/Spouse forms along with Family Forms again.
7) Photo graphs of all family members along with principle applicant.

Remember, If you get any of above documents in URDU, you will be needing notarized English translation along with certified copies of original to be sent.
8) As I was making documents with Nadra, I also applied for Family Tree document. Though it is not a requirement from VO but I had it to have another valid document. (You never know when you may require it in future once you leave Pakistan)
Another bit, as I explored documents needed when one lands in Canada after getting Visa Stamp. All your legal documents, Degrees, transcripts or any other document which you may wish to carry to Canada, should be available with English translation (Notarized copies). So my advice would be to start English translations of your all document at this stage as you are already waiting.

This whole process took 2 months with everything done with urgent fee at Nadra and Passport office.

Best of Luck.

No, my last BFD was 31 MAR 2013 but my CAIPS2 shows that my file was brought forward on 23 MAR 2013. I had later contacted them on 10 APR 2013 to inquire about medicals and they sent me MR the next day by mail. Yes, no e-medical for me. Today I received the meds by post and did the medicals too on the same day. Here in Riyadh, no appointments needed for medicals:). It cost me 1950 SR for 2 adults and 2 kids. I don't know how much medicals cost in Pakistan.

great -- all the best :)

Not so sure dear. But you can easily dig out this info from website. When the time will come, VO will tell you about all details.

mirza_g, i want to make some corrections in the list of documents which you have mentioned because i have also gone through the same process of adding my newly born baby recently. All you need is:

  • The related forms for the newly born only. Send the updated forms of other family members only if the CHC has requested you to submit it otherwise, there is no need to submit the updated forms/docs of other family members.

Bank draft of CAD $ 150 in favor of "Receiver General For Canada"

Submit only NADRA's birth certificate and no need to submit the birth certificate issued by the hospital because that one is only issued as to certify the birth in a certain hospital and then it is used by NADRA to issue the birth certificate which is the only acceptable birth certificate and the one which is issued from the hospital has no worth. Remember this point that you have send the original birth certificate to the CHC and not the copy and this is the only original document which you will send to them.

There is no need to submit Form-B (Nowadays, it is called Family Registration Certificate or FRC) as it is only and only used within Pakistan for different purposes.

Only send a Photocopy of the passport of the newly born and dont send the original passport.

Photographs are mostly not required so don't send it unless and until clearly demanded by the CHC.

So I just completed my medicals from Alwarood Medical Center on Olaya Road Riyadh by Dr Sameh Ibrahim. They charged us 1950 Saudi Riyal for 2 adults and 2 kids under 5. Physical Examination+Blood test+Urine test+Chest xrays for adults. Only physical check up for kids. They gave me Aramex tracking number and the doctor said medicals will reach LVO by next week.

Congrats bro.
