It is called form b in salees english listing down all your children. Hope it helps

No SMSH they didn't ask it with PPR. I just needed that for some diffrent purpose.

Ok - but bro -- this is my first child and if the child is born outside Pak then there's no B or Bay form. I searched through the internet to find out the Long form thing and got an idea that it's a type of birth cert mostly issued in Canada /USA. Well... I guess I will send a photocopy of my baby's bc to them

Dear, I have added my new born recently with my application, made demand draft of his application CAD 150 and sent all docs passport, birth certificate, photos, DD to VO in dec. Hope your question is answered.
