1. New Application for Permanent Residence IMM0008 form for the principal applicant.
2. Schedule A – Background/Declaration [IMM 5669]
3. Supplementary Information - Your travels [IMM 5562]
4. New Additional Family Information Form IMM5406 for yourself and each family member over the age of 18.
5. Copy of passport title page for each applicant. If any of your dependants are included in a parent’s passport, please ensure that you apply for an individual passport for each person. Please note that each person will require their own passport at the time of visa issuance.
6. For all applicants with any military service, please provide a copy of the Military Card or Military Exemption Card.

Plus Documents and Information Required (to be submitted by mail only)

X original UPDATED of police certificate(s) issued within the last three months by the authorities of the following country(ies) for:

principal applicant - country : DUBAI
spouse - country : DUBAI

I was just trying to go through your link, and somehow landed on a thread, where 2 guys were discussing/hoping that the following noc is there in MI-4.

I am now in final stage to complete IMM0008 document , I have left UCI number as blank, bcs I have only my file number , i never applied for CAIPS notes.
Can I write my 2nd AOR Number in UCI Coulmn? please guide me
 
The AOR sent to you by LVO should normally have the UCI number (tagged and defined as UCI) Mine did not change when PVO sent me the medical request.. Also remember that in most cases the UCI number for the principal applicant and its dependents would be different.

I am not sure if it would be ok to leave it as blank, may be some seniors might advise you in this regard.

I also checked the first correspondence received from Centralized Intake Office (CIO) and noticed that it has the same UCI number. So probably this number is permanent and will not change so it is safe to use the UCI number available with you. Good luck.

You can keep it blank. But don't forget to clip it with another plain paper having your file number as per ur 2nd AOR.
