The onliner receipt has empty column of DoB, Client Identification, Address. I want to ask that

Should I fill these columns by hand, scan it and then send to CHC

OR

Should I email as it is?

Luck play a very big role at this stage.
I got PPR after 11 days of medical and my brother got medical after 2 weeks.
I wrote to CHC London after a week of medical and asked for the next step. I was told at that time that when they will recieve medical, then they will advise me.
I think that after two weeks of medical, if you write them, hopefully you will have a good news Inshallah.

You will get PPR by Friday September 12,2014 - Insha Allah
