Surnaname and lastname issue

Hi ,

As per the passport I have surname "Pallikadavath" and given name as "Kishan Sivadas". But in all my certificate it only "Kishan Sivadas".
Now when I will the fill the Express Entry profile do I need fill - "Give the name" as Kishan Sivadas and surname "Pallikadavath".
Or should I fill given name as "Kishan Sivadas" leave the surname? (But the surname is mandatory).

Or "Give the name" as Kishan and surname - Sivadas.

Give your name as it appears on your passport and attach a letter of explanation to explain the discrepancy with your other name.
