Supplementary Information: Your Travels IMM 5562--From the US but live in Canada

I am not sure how to fill out the IMM 5562 form (Supplementary Information, Your Travels), as it asks to list all trips "taken ouside your country of origin or of residence" for the last 10 years. I am an American citizen living in Canada. Do I need to list trips taken from the US to Canada (and vice versa), as one is my country of origin and the other my country of residence? What about trips taken before I became a resident? This question isn't very clear! I have probably close to 50 trips between the two, as I was in a long distance relationship between Boston and Montreal and it will be nearly impossible to reconstruct them all.

List all trips taken in the last 10 years - if that includes trips from the US to Canada and now Canada to US, provide them.

You can request your travel records from CBSA and their US counterparts.

You should provide the following information your request: full name, date of birth, gender, country of citizenship, passport numbers(expired and current), vehicle license number, etc. Enclose copies of the data page(s) of your passport, driver's license and vehicle registration.

Hi , I'm also confused on how to fill the IMM5562, Please suggest howto fill the form in below case
I'm from Asian ountry and,
My first in last 10 years was from Mar/2008 to Feb/06.2009 to Canada, in this period i've visited my cousin in New York for 3 days in in Aug/2008. so should i fill the form like below

From To Length Destination Purpose of travel
03 2008 08 2008 181 days Toronto CAN Work
08 2008 08 2008 3 days New York USA Visited my Cousin
08 2008 02 2009 159 days Toronto CAN Work

Also I've visited almost 25 times to US in this 2 years and currently living in Canada from Aug/2010 to till date

For example if i visited US fro 3 days from Canada Oct/2010 and Dec/2010 for 3 days then should i mentioned may dates like below...?

@Bhanavig, you can fill the form like that. For the last row, you can leave it like that until you print it. Then you can write "present" in the "To" section. So it will be 01-2011 to present

If u are an American, then any trips made between America and Canada is not required to be listed. As america is ur country of origin and canada is ur country of residence.
For instance if u travel Japan or china u must list that

Hi, I also have a question regarding the same form. I am working in Canada since 2010 but am a Indian citizen. So do I have to list this period of work in Canada too in this form? Do I also have to list the trips made to India (which is my country of origin) during this period?

Hi guys,

I have never traveled any where rather than the country I am from and Canada... But to travel back and forth, I had my stop over in other countries but I have never left the airport. Should I simply write I did not travel?

Hello,

My wife is French national and due to her work was traveling a lot within Europe. Since her travels were within the EU (European Union) countries, she does not have any stamps in her passport. I was wondering how to fill this "Your travels" form then. From memory it is close to impossible, since there were probably 50 flights to London, Amsterdam, Madrid, Rome, Poland, Germany and etc.

Will be grateful for an advice how to fill the form.

I'm staying in Canada from 2009 on work permit. didn't travel outside Canada since then.

Supplimentary information form clearly indicates to provide travel info "taken ouside your country of origin or of residence".
so I updated as "didn't travel".

I feel travel information (from --> to) (to --> from) country of residence and origin shouldn't be provided.

In the same boat as you...I'm a European national and when traveling throughout the EU, barely have any stamps. Luckily, my trips also included entry into non-EU states, which then meant stamps. Nevertheless, I probably will not be able to provide a full travel history that is accurate.

And I may be wrong here, but my plan is to attach an explanation letter citing the lack of stamps received when in EU. In addition, I will fill out the IMM 5562 as best I can or can recollect. If we have at least something on the form, it will look like one made an effort. With the explanation letter and the travel noted in IMM 5562, I think it should be fine. This is not something you can control, and I think the VO will understand. But make sure you list all other travels that were documented in your passport, and those you can remember.

Maybe seniors have a different approach. This is just my take on it.

Thank you CEC2013 for your reply. I planned to do exactly as you suggested, e.g. write an explanation letter in addition to anything we can remember. I hope it will suffice.
Unless someone knows a better way?
