Supervisa Query for Medical Surveillance

I applied for parents super visa and got a PPR:
Father was furthered for medical test and he also got an additional form IMM 0535 along with PPR, in which there is an address box which states, address where you intend to reside in canada and is prefilled with home country address, is it ok?
And what is to be done with this form??
Has any one else received this form? Please guide

You will be able to update the address by contacting the Public Health Liaison Unit at IRCC.MHBSurveillance-SurveillanceDGMS.IRCC@cic.gc.ca if you’ve changed your Canadian home address, phone number or email address. Make sure you provide your full name and unique client identifier (UCI) number in your email.

Thank you for response, but we have to print this form and take it to officer at the point of entry, and address in form is my concern, as it is home country address
