the above is one of the requirement to be eligible for application. However, How can the insurance be valid for 1 year from entry when we don't know when "entry" is, and processing time is 250+ days. Can someone please shed some light?

For example your intended travel date is 19 April 2022, then you buy a policy with effective date on that date, If the applicant cannot travel at that time, you can go over with the insurance company for a full refund.
