submitting application and Letter of explaination

Hi friends,

I have couple of questions, Please help me out.

1) Do we need to send hard copies despite submitting an online application? If yes, Do we need to include any other thing other than what we submit online and what would be the "to address".

2) I have different names on my bachelors degree and Masters degree. Is there any template to explain this situation in LOE.
example:
Bachelors Degree: A. Arjun
Passport and Master's Degree: Arjun Aaaaaa

I believe no hard copies are needed. As for the name, my hisband is in similar situation, on one of his diploma they added his middle name. We are going to add a section in LOE "Name clarification" and explain why the difference.
