Student Questionnaire - has anyone filled this form ?

we need to fill this form by hand or print it electronically?


Also,can someone direct me to this form location..

To fill it, we need to copy it and do the editing? Is it so?

Hi,
Student questionnaire form needs to be filled by hand. You need to take printout and then fill it by hand.
The 2 forms to be filled electronically are Visa application form and family information form.

What about VFS consent form? It was written next to Name (printed)___________
So, what it meant,we need to print the name or print the form first and then fill it by hand?

While filling student quetionnaire form, i came accross this question

4) If your course of study includes English as a second language (ESL) component, please explain how long it will take to complete English as a second language (ESL) requirement and how much it will cost.

Can anybody give sample answer to this question?
