My parents are here in canada on oct 23rd 2018 on visitor visa and there return ticket is booked on april 17th 2019 i.e less than 6 months.
But in december 2018 they went to usa to visit my brother for a week and came back to canada again on jan31st.
This means legally then can stay in canada for another 6 months i.e until jun 31st.

my question is
my parents are again planning to visit my brother for a week again in april 2019. If they came back to canada in april again --are they allowed to stay in canada for another 6 months starting form april without extension of the visitor visa.
are there going to be any complications during immigration. should they require any medical tests to be taken?

They will be allowed to stay however long CBSA allows them to. Given the multiple entries in a short amount of time, CBSA may limit their stay.

I don't have similar experience but I'm looking from some answers based in you experience since I'm similar situation:

When your parents when to USA and came back, were they asked for a return ticket?

My mother 6 months maximum stay is coming due May 2019, we were planning a trip to mexico but now I don't know if we should do the trip or apply for the extension instead? I'm afraid that going to Mexico looks suspicious or a way to re-enter to get an extension and then, when coming back they give her a stay less than 6 months.

Any visitor extension will lapse once your mom leaves the country to Mexico. If she intends to visit Mexico then it is pointless applying for an extension.

She will be assessed for a new entry (new stay of maximum 6 months) when she re-enters Canada.
