Statutory Declaration of Common Law union needed for OWP application?!

I am sponsoring my common law partner and he is also applying for an OWP.
On the document checklist (IMM 5556 F), it says

"the following forms must be completed, signed, and dated:
-application to change conditions, extend stay etc
-use of a representative
-authority to release personal info to a designated individual
-Statutory Declaration of Common Law union

Two questions for you guys:
1) Do we just write N/A next to "use of a representative" and "authority to release personal info"?
2) It is mandatory to fill out the declaration of common law union/get it signed by a notary?

*there is confusion because on the regular application checklist (not the worker one), it says the declaration of common law form is not mandatory.
