I received a request for some documents including a Statutory declaration form regarding non accompanying dependant. What is the format for this form? I had created an affidavit and declaration 2 years ago and submitted my medicals as well. Now request has come for medical again and this declaration. Does anyone know what is the format for this declaration? Can i Submit previous proofs of communication with ex spouse? Im really confused and i have only a month time.. pls help if anyone going through same...
