state level police certificate from Illinois

Hi Everyone,

I really hope someone can help with me with this. My wife is currently in canada and we are going through the PR process. We received an correspondance stating she is require to provide a state level police certificate from Illinois with fingerprint.

I found this link https://isp.illinois.gov/BureauOfIdentification/Fingerprint

now the issue is that when we request an out of state form, it doesn't take our Postal Code. anyone was able to by-pass that or do we need to call at (815) 740-5160 to get the form mailed to us?

Looks like zip code is not required (no *) and you also can’t put a Canadian province since State just says “Non-US”. I’d suggest trying putting down City, Province, and postal code in the blank for City. Hopefully that works (if the system complains, try removing comma between city and province).
