spouse open work permit once on visitor

Can spouse do medical from back home and when she arrive in canada apply for open work permit? Actually its required in Health related field.

Anyone did it? any suggestion on it.

Yes. Medicals can be done anywhere in the world. As long as they are linked to her UCID, her work permit will remove the clause about health fields.
