That's an option but then all our mail (including the five thousand catalogs my wife subscribes to) will end up in my buddy's mailbox, lol.

I think I will email the Manila office and see what they say.

Medical Exam: Is there a form to prepare before going to the hospital? This is to be included with the application package right? My wife called severance hospital today and they asked he if she had the form? They do require passport photos. Please direct me to the form if there is one.

Technically that form is NOT required anymore, as of November 2012. http://www.cic.gc.ca/english/resources/manuals/bulletins/2012/ob471.asp
With the release of the new GCMS 4.0, it should be noted that all previous versions of the IMM 1017 form as well as the Appendix C will no longer to be used.
IMM 1017B: Upfront Medical Report: Client Biodata and Summary (not available on our website as panel physicians will have these forms in their office. With the implementation of the new IMM 1017B, Appendix C will no longer be required)


HOWEVER... CIC does a horrible job of communicating changes and new requirements to their panel physicians around the world. So many are still insisting on the Appendix C simply because this is how it used to be done. So instead of arguing with the doctor's secretary, it's best to just fill it out and give to them if they want it. As long as you get the IMM1017 form at the end of the medical, that's all that matters.
 
That's the form my wife took as well. Severance should be able to tell her about the photo specs.
 
Yeah, Severance is asking for a form so i guess they are not up to speed with the cic updates. They want 2 passport photos wife just said. thanks.

Still waiting for option c to arrive in the mail. Got the medical form ready to get done.

9 immi photos as per guide specs: anything have to be stamped or written on back?

2 medical photos as per guide (passport size): anything have to be stamped or written on back?


Wife had these done on different days. Do they all have to be the identical?

I wrote nothing on the back of ours. I doubt it matters if the photos for the medical are different than the photos for the application.

As for our application, my sponsorship was approved on April 4th. The file is being transferred to Manila for the long wait. We are leaving for Canada two weeks tomorrow.
 
Bartjones: How long did it take to hear after you sent the application? Was it via mail or e-mail? thanks! Yes, may as well wait in Canada on Manila. How did you handle the mail forwarding thing after. Just asking in case we have to do the same later on.
 
Can't remember a phone number for sponsor questionairre for previous address/phone number section. What to do?

Sponsorship approval came 28 days after receipt. They acknowledged receipt by email a week after the courier delivered the package, but sent the sponsorship approval by regular mail. I don't have the letter yet, but expect it will arrive this week. I only know it was approved because I called CIC and they told me.

I managed to change my address on line. I sent an email to Manila to change my wife's address. They acknowledged receipt of the email but haven't actually changed the address yet. I'm keeping an eye on that.

I had this problem too. I just typed "can't recall phone number" in the box. It was fine.
