Can a mother with PR status sponsor and eventually bring a 19 year old child to Canada without the consent of te father? Mother and father are separated but not legally.

The child cannot be sponsored because he/she is not under 19, unless the child has a mental or physical illness that requires constant care from the parent.

Even if the child was under 19, you wouldn't be able to do it without the other parent's consent.

What would be the reason that a child 19 years of age was able to come to Canada without the father's consent? Is it possible that the father's signature was forged?

No it's probably because a child of that age is now an adult, not a dependent child, so he/she wouldn't need permission anyway. If the child was under 19 and you try to get him/her here without the other parent's permission, it's pretty much like child kidnapping.

As explained, the child is too old to be sponsored by their parent. The child will have to qualify to immigrate to Canada on his/her own. No parental consent is required.

Actually when the application was submitted the child was pretty much a minor. But when the visa was finally issud and the child came to Canada he was already 19. Father is wondering how it happened when he never signed anything.

By the time the child came to Canada, he was no longer a minor and no longer needed parental approval to move. There was nothing for the father to sign. The child was an adult and free to move to Canada without parental approval.

So the proper processes were followed and nothing is wrong. The father did not have the authority or control to stop the adult child from moving to Canada.
