We received ********* application for permanent residence on September 6, 2012.

Medical results have been received.

What doesn it mean??? They rececvied the application in Islamabad or still working inside in Canada???

As said in the email, your application is now in Islamabad, and they have the medical results in hand.
Average processing time for Islamabad is 26 months so you do still have a while to go..

The next step is waiting. Often times you do not hear from the visa office until they are nearing the end of your application.
 
For Islamabad it is quite a long time. Many forum members who apply for spousal sponsership for Islamabad wait over a year to hear something from visa office.

It takes about 15-19 months for full approval.
