Some questions about submit documents

We live in New York State, but not American. My family (3 people) would like to make a travel to Canada recently, we made a application online and got a request letter the day before yesterday.

There are 4 questions we are not sure when we prepared the documents as requested,

1, About Fees: any applicable visa application centre (VAC) fees, such as passport transmission fees;

The service fee seems to be $19.90 USD according to the following link
https://www.vfsglobal.ca/Canada/USA/Service_and_Service_Charge.html
because we need not to provide our biometrics, and we are not submitting paper applications, and we do not need the self service workstation or any other service.
So we just pay the Package transmission fee($19.90), Is that right?

2,About Packages:
How many packages do we need, 1 famaily as a whole package($19.90 total) or 3 people need 3 packages($19.90*3=$59.7 total)?

3,About mail the package:
Seems like the VAC in New York has moved to
Canadian Visa Application Centre – New York
145 West 45th street, 4th floor
New York, NY, 10036
U.S.A.

or
(https://www.vfsglobal.ca/Canada/USA/contact_us.html)

The request letter says that the passport must be submitted to the above address along with the request letter. Also, the service fee will be payable to TT Services (https://www.vfsglobal.ca/Canada/USA/Service_and_Service_Charge.html).

The old process was to mail two packages
- First with VAC service charge of 32.27 payable to CSRA Consular Services of Boston
- Second with passport, passport request form and other documentation to New York VAC

The new process seems to mail a single package
- With VAC service charge of $19.90 payable to TT Visa Services Inc., passport, passport request form and other documentation to New York VAC

Is that correct? I should not be mailing a money order to CSRA in Boston anymore, right? Please help.
I can not find the detail instruction about it anymore.

4,For the VAC Consent Forms, which page should I provide?

It seems I only needed to fill the page2 .
because we need not to provide our biometrics, and we complete all the things by ourselves not by agent or CVAC staff or others.
Is that right?

Kindly read VFS global google review before sending your passport, they have lots of troubles,
I sent my passport 10 days before, I don't know where it is now, they have only one email listed on their website which never reply back and customer service does not have enough rights to give you any information. Once you mail your passport there is no way to contact with them except go physical new York.
