The common in law union,use of a representative forms,and medical condition statement for adopted child doesn't apply to us, should we still print them out and write not applicable in the spaces, or just leave the forms out completely?
 
No need to include the forms which are not applicable
 
some people printed them and wrote Not Applicable...for me that's waste of paper...on the checklist i will just write Not Applicable for the ones not concerning our situation (many did like that too and i didn't hear of any complaints)
 
Thank you, I just don't wanna get held up because I didn't send in forms not applicable to me =/

Not if you DO NOT have a common law, rep and an adopted child.
