Signature on Use of Representative form

Regarding the "Use of a Representative" Form (IMM5476). My wife would like to have me as her representative - I am the sponsor. Since I am the sponsor and the representative should I be singing in both Section B and D? Or just section B? I've read stories about troubles signing in the wrong place or not singing in the right place so I want to be sure to do this right.

i would imagine you would need to sign in both locations because technically you are both, the sponsor AND the representative. i didn't use the form, so i'll defer to other people who did...
