Siblings Points - Affidavit for Parent's Name Difference in Passports

Hi Guys,

I am claiming points for my canadian citizen brother.

My passport has parents' names without surname.
but my brother's OCI card has parents' names with surname.

Kindly advise next step. If its an affidavit, kindly share the format.

Visit the nearby Notary, they have all the formats.
Affidavit will be enough for the name problem.
