Should student from India fill up form "APPLICATION FOR TEMP RESIDENT VISA" along with other study visa application documents?

I don't understand what this form is about.

I think this form is not in your checklist. The questions that the form has is included in the updated IMM1294.

You need to go to VFS Canada India website and find your checklist ( there are three, figure out where you belong ) and forms under the student section. These are the ones you need to submit as part of your package.
