Schedule A – Background Declaration form
I am doing job with a company and is ongoing. Moreover I am also an active member of an international Organization. What to write in the “TO” date column? For the following questions:
1-Question # 8 (Personal History)
2-Question # 9 (Membership or Association with Organizations)
Should we leave the “TO” date column blank or write date on which form was filled ? No text like “Till date / To date” etc can be written as it only accepts the date format and no text. \
3- Question # 9 - What to do if the space is not enough to write the complete name of Organization through computer typrwriting? Should we leave teh cell blank and manually handwrite the info after the print ? or should we attach separate sheet for it ?
