How can I get this document?

I looked at the website, and there seems to be a form for only Access to Personal Health Info, and it is not for summary, but rather for requesting detailed visit info from individual clinics or hospitals.

I also got the same request today(Manitoba applicant), I filled the PHI access form and sent it via email, luckily got the reply they said it will take 1 week.

Did you try to pull some info from MySaskHealthRecord https://www.ehealthsask.ca/MySaskHealthRecord/MySaskHealthRecord
Mine shows last 3 years of doctor visits, prescriptions, lab tests

I'm in the same boat. we have only visits related info in MySaskHealthRecord. they are asking for claim history.

well if you didn't had any claims then there isn't any to give to IRCC. Just send them whatever you have.

Its the same for me. I also only visited couple of times in last 4 years so whatever I have is I will submit provided IRCC asks me for these documents.
