I'm going to file a study permit application soon. Just need to confirm the forms that are required because of special instructions from Pakistanis:
1) IMM5829E Documents Checklist Pakistan (This is the document checklist which states a few additional things are require a few additional forms to be filed)
2) IMM5483E Document Checklist
3) IMM1294E Application for Study Permit
4) IMM5257B_1 Schedule 1
5) IMM5562E SUPPLEMENTARY INFORMATION (TRAVEL HISTORY FORM) This is the form I'm confused about
6) IMM5645E Family Information
So I'm confused about the IMM5562 form. Is it required by CHC Pakistan? Says so on the checklist for Pakistan IMM5829 (but doesn't specify the form with its number). Any one filed their application with this form? There is no field for sign and date.
Any other forms I should be submitting with my application?
