requesting notes

I know I've read about how to request notes, but I cant remember where I read it,

im just worried they tried to contact us and it went to junk mail, or we didn't receive a letter...

so we want to request some notes just to make sure, thanks in advance!

There are some updates to the form but it relatively same altogether - made a video ( click on my signature ) or u can follow the instructions in above post
