Requested medical even after sending the upfront medical

I submitted my application after receiving my ITA. Me and my wife both in canada.
I submitted the upfront medical document while applying.

Now after almost 28 days, CIC is asking a medical test for my wife. What should I do? Do a case specific inquiry?

I kindly request your opinion/experience regarding this.
