regarding information form

hi,
I have doubt in filling family information form , what is to be written in the present address column for every member - whether the home address or work address ?

One more thing i am applying for second time so is it necessary to mention UCI no. on main application as i have mentioned my rejection in column requiring details of rejection ?

1. Home Address preferably (Where they can be contacted or any confusion can be verified ((though its a rare rare chance)) )

2. Yes you should. It is mentioned in the Form Filling guideline that you should enter your UCI. Only a first time applicant will not write a UCI.
