Reference Letter Details

Hi,

I have received an ITA and I'm gathering my documents. I would like to know if the Reference Letter from our present and past employers should include the NOC Code.

Also, does the company address have to be present only in the letterhead or does it have to be mentioned below the supervisor's signature, name, company name as well?
