Reason for Refusal?

I got a refusal letter last Dec. 12, 2013 and the letter has a general information only, which saying that the officer is not satisfy and I did not meet the requirement and so my work permit application was refused.

At the back of the said letter is a listing of reasons but none of these has tick mark in the boxes. In short, I don't know the reason why my application was refused.

Can somebody help me... I want to know the reason so that the next time I will apply again, I can make sure that I will qualify on their requirement.

Visa Office: Manila
Application Submitted: July 03, 2013
Required for Medical: Sept. 10, 2013 - took the Med. Exam on Sept. 12, 2013
Application Refused: Dec. 12, 2013

My Medical exam is clear, how come they refused my application. Is there a possiblity that It was mistakenly sent to me because of the volume of applications?
