Hi Folks,

Anybody ever filled up Schedule A Background / Declaration form ?

In personal history section it asks for "From" and "To" date of my activity. What should i put in "To" field when i am currently in the job ?

Please suggest.

As I had to fill out this form as well I used current month and year 2015 -09

Hi,

I found that 3rd line usually is medical results received. But my 3rd line and 4th line appeared quite different today:
Permanent Residence
1.We received your application for permanent residence on July XX, 2014.
2.We started processing your application on February XX, 2015.
3.We transferred your application to the New Delhi office on November XX, 2014. The New Delhi office may contact you.
4.We sent you correspondence on March XX, 2015. If you have not yet provided the information or the requested documents, please do so as soon as possible. Please wait until you receive the correspondence before sending us additional information, as the correspondence will outline all information that is required.
5.We sent you medical instructions on March XX, 2015. To avoid delays, please provide us the information requested in the letter as soon as possible. Please consider delays in mail delivery before contacting us.

I sent medical results in march itself. Then I received a letter from CIC for additional medical tests in Apr. I completed those in may.
In my status on CIC website, first two line was there since march and from line 3 to 5 got updated today.

Can you please suggest, if its normal? I didn't received anything since i completed requested tests in May.
It has been more than a year, since my case is in progress. I am loosing hope.

Can you please provide your expert advice what to do at this point?

My toddler daughter use to have medical issues in past. CIC asked, letter from her neurologist, pediatrician and from school to get her current condition. I provided all of them and they were pretty positive.

For travel history, I am currently in US since 6 years. Travelled to India for vacations 4 times.
I sent my PCC from US with original application in July 2014 and they asked PCC from India with medical tests in March 2015. I provided that with medical results in March.

Yes it's normal.
Before July the "Medicals received" used to be third line but it's now the 6th line. They have updated the criteria of information share on Ecas. U should be waiting for 6th line now.

I have paid RPRF online through my friend's CC. HIs name is appearing on the receipt generated. I hope this will not be a issue. I have to fill my address, phone number and date of birth by hand on the receipt. Kindly clarify. thanks Medicals have been booked to be done on 18 september 2015
