Questions regard PR renewal Process

Hi all.

I got my initial PR card in June 2008 and is set to expire July 2013. I have already met my residency obligations as I have been working in Canada during that period, with only leaving the country for small vacation durations. I am planning to apply for the renewal card by the end of January 2013. Here are some of the questions i have

1.The only applications I need to fill are IMM 5455 and IMM 5444 or are there additional applications?
2. I am planning to leave Canada by August, will I get the PR card renewed by then?
3.The fee for renewal of the PR card is 50 dollars, are there any other fees?
4. To prove I was in the country all I need is my tax slips, or is there any other documentation needed ?

Another question to add

what would be the validity of the renewed PR card, another 5 years?

CIC will issue a 5 year card unless investigating your residency (1 year card) - for processing times and required documents see the link below:
