Question regarding IMM 5669

Hello everyone! I'm having a bit of trouble validating IMM 5669 and would like your help. In regards to the Form, I am unable to validate because of some error in question 10 concerning where my wife lived for the past 10 years. My wife was only able to give me two addresses because those were the only two addresses she has lived for the past ten, and still lives currently at one of them. Unfortunately I am unable to validate the form because it seems they want more information since it's telling me to not leave any gaps in time.
What should I do?

Make sure there are actually no gaps. For example, if she lived at one address from September 2004 to March 12, 2015, and then another from March 13, 2015 to today, the form may believe there's a gap because she had to spend the evening of March 12 somewhere.

The end date of one address should match the start date of the next.
